Requesting Network Account Change

This page is designed for use by L&S faculty and staff seeking to grant access for themselves or others to log-on to an L&S IT Office-supported computer. Users must already possess a UWM email address; this form is not a replacement for the traditional New User form that creates UWM email accounts. The College IT Office requests that those seeking changes to network accounts provide several details necessary to identify the affected accounts and their department membership. Filling out the form below will help ensure the most rapid and effective completion of your requested account change. Please make sure to specify whether the account change is a request for addition or removal of account rights.

NOTE: Only computers running Windows XP are currently supported by the College's network infrastructure, which permits centralized account management. For all other operating systems, and for computers that do not authenticate to the network, the account change will need to be made directly on the affected computer. In such cases, you will be contacted to set up a service appointment.
