If so, you MUST complete a UWM driver authorization form at
the following website:
http://www.uwm.edu/Dept/EHSRM/RISK/DRIVING/. For department,
use "Mathematical Sciences."
Due to legal issues, UWM's risk management department may
require that non-authorized drivers will not be allowed to
participate in the chase. If so, your name
must appear on the authorized list before you will be allowed to
participate.

Meteorology? Choose yes if you are currently taking one of
these classes. These classes are extremely helpful for
understanding severe weather forecasting. However, they
classes are not required in order to participate in the chase.

NOTE: On the back of this
application, please note any conflicts or scheduling concerns (such as
finals, graduation, family events, etc). This information will
help us select who will be in each group.
PART IV: Equipment
The Atmospheric Science Club will provide receivers for GPS and radar
data to each chase group. This hardware is worth a large sum of
money, and all participants are asked to treat the equipment with
extreme care. All other equipment is provided by individual
participants. Each participant assumes their own risk for any
equipment and personal property brought along on the chase.

PART V: Disclaimers
I agree to release the Atmospheric Science Club and the University
of Wisconsin-Milwaukee from any damages to personal equipment or
property that may occur while participating in the 2006-2007 Storm
Chase.
I also agree to abide by all rules and regulations set forth by
the Atmospheric Science Club, the University of Wisconsin-Milwaukee, and
the vehicle rental agency. These include, but are not restricted
to: codes of conduct, driver authorization, and storm chasing etiquette.
If other legal disclaimers are required, they must be signed in
addition to this application before participation can be accepted.
