Family Status Changes: Marriage

State Group Health Insurance

You can change from single to family coverage to include your spouse (and stepchildren) provided you were not eligible for family coverage previously. Your application must be received by the UWM Benefits Office within 30 days of your marriage. Coverage is effective the date of your marriage.

If you were enrolled in family coverage before your marriage, you must complete a Health Insurance Information Change Form ET-2329 to report your change in marital status, to add your spouse to the coverage and, if applicable, to change your name.

If you marry another State employee, you are eligible for either one family contract or two single contracts. If you need coverage for children, one family contract is necessary. Even if there are no children, one family contract may be desirable due to the availability of your sick leave credits upon your death. State Group Life Insurance: Minnesota Life Insurance Company

You can add Spouse and Dependent coverage (provided there were no eligible dependents before your marriage) by completing an application and submitting it to the UWM Benefits Office within 30 days of your date of marriage. If you do not apply within the 30 day enrollment period, you can apply only under evidence of insurability.

If you had Spouse and Dependent coverage before your marriage, due to covering eligible dependents, your spouse (and
stepchildren) will automatically be covered without submitting an application.

Individual and Family Group Life Insurance: Minnesota Life Insurance Company

You can add coverage for your spouse (and stepchildren) by submitting a coverage change form (UWS-8) to the UWM Benefits Office within 30 days of your marriage. If you do not apply within the 30 day enrollment period, you can apply only under evidence of insurability. Stepchildren are automatically covered if you have children coverage in effect prior to date of marriage.

Accidental Death & Dismemberment Insurance: Zurich Insurance Company

To change from "Employee" to a "Family" plan upon your marriage, submit an application to the UWM Benefits Office. There is no specific deadline by which you must apply. Coverage is effective when your application is received by the UWM Benefits Office.

Major Medical and Dental Insurance: Epic

You can change from single to either 2 person or family coverage by submitting an application to the UWM Benefits Office within 31 days of your marriage. Coverage is effective the date of the marriage.

If you were enrolled in family coverage before your marriage, you must complete an application to add your spouse (and stepchildren) to your coverage.

Dental Insurance

Unclassified Employees and Non-Represented Classified Employees
You can change to family coverage within 31 days of your marriage. Complete and submit an application to the UWM Benefits Office. Coverage is effective the date of your marriage. If you were enrolled in family coverage before your marriage, you must complete a new application to add your spouse and stepchildren, if applicable, to your coverage.

Represented Classified Employees
Contact your union local regarding requirements and procedures.

Employee Reimbursement Account Program

If you have a Dependent Care or Medical Expense account in effect, you may be able to adjust or cease your contributions. The change you make must be consistent with how your marriage has affected your expenses in these areas. If you do not have an ERA account in effect, you may start one or both. You must submit a completed Family Status Change Form to Fringe Benefits Management Company within 30 days of the marriage.

You also have the opportunity to waive the automatic premium conversion part of the ERA program, or retract a waiver if you had waived this benefit previously. You must take action within 30 days of your marriage.
