The following pages address the affect of specific "family status" changes on your benefits. These family status changes include marriage, birth, adoption and legal guardianship, divorce, death of an employee and death of a spouse or dependent child.
Beneficiary Designation
You may wish to change your beneficiary designations for your life insurance plans, the Wisconsin Retirement System and your Tax-Sheltered Annuity/Deferred Compensation accounts whenever a family status change occurs. Beneficiary forms for your life insurance plans and the Wisconsin Retirement System are available from the UWM Benefits Office. If you have a Tax-Sheltered Annuity account, call or write your vendor to request a form. If you have a Deferred Compensation account, call National Deferred Compensation, Inc. in Madison at (608) 256-6200 or toll-free at 1-800-257-4457 outside of the Madison area to request a form.
