Family Status Changes: Death

Wisconsin Retirement System

One of your survivors should contact the UWM Benefits Office or the Department of Employee Trust Funds at (608) 266-3285 to inform them of your death. Employee Trust Funds will notify your designated beneficiary(ies) regarding your death benefits.

State Group Health Insurance

If you have family coverage at the time of your death, coverage for the surviving spouse and/or dependents can be continued for life. Premiums are paid by utilization of sick leave credits, a deduction from a beneficiary annuity or direct payment to the insurance carrier. Sick leave credits are lost if an employee dies while covered by a single contract. See the Accumulated Sick Leave Conversion for Health Insurance section of this booklet for more information.

Income Continuation Insurance

Upon your death, one of your survivors should notify the UWM Benefits Office with the date of your death if you were receiving Income Continuation Insurance benefits.

State Group Life Insurance: Minnesota Life Insurance Company

If you were insured by the State Group Life program, one of your survivors should notify the UWM Benefits Office upon your death. The UWM Benefits Office will mail a claim form to the Department of Employee Trust Funds and to the Minnesota Mutual Life Insurance Company. Your beneficiary should mail a certified death certificate to Employee Trust Funds, P.O. Box 7931, Madison WI 53707-7931. If your spouse or children were insured by your State Group Life insurance, they are eligible to convert coverage to individual policies by contacting the Minnesota Life Insurance Company at (608) 277-8690.

Individual and Family Group Life Insurance: Minnesota Life Insurance Company

If you were insured by Individual and Family Group Life, one of your survivors should notify the UWM Benefits Office upon your death. The UWM Benefits Office will mail a form to your beneficiary to be returned with a certified death certificate. If your spouse was covered, coverage is continued one year with no premiums due, then may be converted to an individual policy. If children were covered, coverage continues until age 25 with no premiums due. After that, coverage may be converted to an individual policy up to five times the amount of coverage which was in effect.

University of Wisconsin Employees Inc. Group Life Insurance - Mutual Service Life

Upon your death, one of your survivors should contact the UWM Benefits Office to file a death claim. The UWM Benefits Office needs a certified copy of the death certificate to process the claim.

University Insurance Association Life Insurance: Minnesota Life Insurance Company

Upon your death, one of your survivors should send a certified death certificate to the Minnesota Life Insurance Company, Attn: UIA Group Life, Post Office Box 259708, Madison, WI 53725-9708 with a memo indicating UIA coverage. (Note: Although this insurance, like the Individual and Family Group Life, is with the Minnesota Life Insurance Company, the claims processing is separate, and two death certificates are needed if an employee was insured by both programs).

Accidental Death & Dismemberment: Zurich Insurance Company

If your death was accidental, one of your survivors should contact the UWM Benefits Office regarding claims processing.

Major Medical Insurance: Epic

Your insured spouse and/or dependents are eligible to continue coverage. They should contact the UWM Benefits Office for continuation information. If your death was accidental, one of your survivors should contact the UWM Benefits Office regarding claims processing.

Dental Insurance

Unclassified Employees and Classified Non-Represented Employees
If family coverage has been in effect, your insured spouse and/or dependents should contact the UWM Benefits Office for information on continuing coverage.

Represented Classified Employees
If family coverage has been in effect, your insured spouse and/or dependents should contact your union local for coverage conversion information.

Employee Reimbursement Account Program

If you had a Dependent Care or Medical Expense account in effect, one of your survivors should call the UWM Benefits Office if they wish to make arrangements to continue or change coverage.

Tax-Sheltered Annuities/Deferred Compensation

Upon your death, one of your survivors should contact your Tax-Sheltered Annuity vendors to discuss payment options. If you had a Deferred Compensation account, one of your survivors should contact National Deferred Compensation, Inc. at (608) 256-6200 or toll-free at 1-800-257-4457 outside of the Madison area to discuss payment options.
