
Throughout this section, birth refers to the date of birth, adoption refers to the date the child was placed in your home for adoption, and legal guardianship refers to the date the child became your legal ward.
State Group Health Insurance
If you have Family coverage, you will need to complete a Health Insurance Information Change Form to add a dependent. Coverage is effective from the date of birth, adoption or legal guardianship.
If you have Single coverage, you can change to Family coverage by completing an application and submitting it to the UWM Benefits Office within 60 days of the birth or adoption, or 30 days of legal guardianship. Coverage is effective from date of birth, adoption or legal guardianship.
State Group Life Insurance: Minnesota Life Insurance Company
You can add Spouse and Dependent coverage (only if you had no eligible family members before the birth, adoption or legal guardianship) by completing an application within 30 days of the date of birth, adoption or guardianship and submitting it to the UWM Benefits Office. Newborns are not covered until they are 14 days old. If you do not apply within the 30 day enrollment period, you can apply only under evidence of insurability.
If you had Spouse and Dependent coverage before the birth, adoption or legal guardianship, any new dependents will automatically be covered without submitting an application.
Individual and Family Group Life Insurance: Minnesota Life Insurance Company
You can add coverage within 30 days of the date of birth or adoption (if no children were eligible previously) by submitting a Coverage Change Form to the UWM Benefits Office. Newborns are not insured until they are 15 days old. If you do not apply within the 30 day enrollment period, you can apply only under evidence of insurability.
If you already have coverage for a child, you do not need to submit a form to add another child. Any new dependents will automatically be covered.
Accidental Death & Dismemberment Insurance: Zurich Insurance Company
To change from "Employee" to a "Family" plan upon birth, adoption or legal guardianship, submit an application to the UWM Benefits Office. There is no specific deadline by which you must apply. Coverage is effective when your application is received by the UWM Benefits Office.
Major Medical and Dental Insurance: Epic
If Family coverage is already in effect, you can add dependents to your coverage.
If Two-Person coverage is in effect, you can change to Family coverage. Coverage is effective the day of the birth or date of placement or court decree for adoption.
If Single coverage is in effect, you can change to Two-Person coverage to cover a child effective the day of the birth or date of placement or court decree for adoption.
You must submit an application for any of the above to the UWM Benefits Office within 60 days of birth, adoption or legal guardianship.
Dental Insurance
Unclassified Employees and Non-Represented Classified Employees
You can change to Family coverage within 31 days of date of birth, adoption or legal guardianship. If family coverage is already in effect, a new application is needed to add the dependent. Complete and submit an application to the UWM Benefits Office. Coverage is effective from the date of birth, adoption or legal guardianship.Represented Classified Employees

Employe Reimbursement Account Program
If you have a Dependent Care or Medical Expense account in effect, you may be able to adjust or cease your contributions. The change you make must be consistent with how the birth, adoption or legal guardianship has affected your expenses in these areas. If you do not have an ERA account in effect, you may start one or both. You must submit a completed Family Status Change Form to Fringe Benefits Management Company within 30 days of the birth, adoption or legal guardianship.
You would also have the opportunity to waive the automatic premium conversion part of the ERA program, or retract a waiver if you had waived this benefit previously. You must take action within 30 days of the birth, adoption or legal guardianship.
