Unclassified Category A Position Description/Questionnaire Forms

When to Use the Unclassified Category A UW System Position
Questionnaire and UWM Position Description Forms*

Beginning in 1996, UW System required institutions to use the "UW Position Questionnaire" (long form) as the official position description format for all Category A academic staff positions. In December, 1999 UWS administration authorized the use of a shorter "Unclassified Position Description," which can be used by institutions in certain situations. The following information provides guidance as to when to use each format:

Use of the UWS Position Questionnaire (Long Form):

This format must be used when the title assignment requires a position rating by Human Resources utilizing the UW System "Title Evaluation Instrument."
  1. For the assignment of a Program Manager suffix level (I, II, or III), or when the strong potential exists for the assignment of a Program Manager title.
  2. To support requests to UW System for non-delegated title assignments- Director, Unspecified (and Associate, Assistant levels); Special Assistant; and Administrative Officer.
  3. For the potential assignment of a title in the Administrative Director and Assistant Dean series, when requested by Human Resources. (The PQ may be helpful in distinguishing a director from a program manager title assignment and/or to determine a scope level- large, medium or small).

Use of the UWM Position Description (Short Form):

  1. For new or redefined positions that will be assigned a generic title in the Professional and Administrative Director series.
  2. For the update of a position description that should be completed for a prefix change/promotion within a Professional title series.

*The Department of Human Resources should be contacted ( hr-contact@uwm.edu) whenever there are questions as to which form should be used.
