These forms are to be used by the supervisor, appointing authority and the newly hired Classified Limited Term Employment applicant for collecting the LTE's acknowledgment statement/signature, the appointing authority certification statement/signature, and other local approvals and related data. Duties of the position must be listed or a Position Description may be attached.

The forms' fields are self-explanatory and capture specific information rated to the LTE, their duties, rights and conditions governed by s.230.26, Wis. Stats., Ch. ER-Pers 10, Wis Adm. Code, and ER 10 Wis. Adm. Code.
