Classified Staff LTE Candidates Log
This is the form to be used by recruiting departments to record the
self-referred candidates for jobs posted at Milwaukee Job Center-South
via the LTE Recruitment Form. It is used to assess affirmative action
efforts and to comply with State and Federal regulations for monitoring
applicant pools. The department completes the form and forwards it for
approval to the school/college/
division personnel representative. The personnel representative attaches it to
the LTE Action Form, LTE Request/Report Form, Employee Self-ID form,
and the W-4 completed for the new hire to the job.
The form captures the person selected, applicant names, gender, race, and referral source data both
individually and cumulatively.
