
The Form B Interview - Selection Summary is to be used by recruiting departments to record the names of candidates for unclassified positions who have been selected for interview, and later to record the name of the candidate selected for hire. It is used to assess affirmative action efforts and to comply with Federal regulations for monitoring candidate pools. The information for Section 1 is supplied by the department. In Section 2 the interview list of candidates is supplied and authorization signatures are required. The Form B is then sent to Human Resources along with 2 copies of the Applicant Log and 1 set of vita/resumes for the candidates to be interviewed.
Section 3 is completed when a final selection has been made from the list of interviewed candidates. Proposed start date and authorization signatures are required. The Form B is then sent a second time to Human Resources for final approval of the selected candidate.
