
A Form A (Request to Recruit) is completed by the person responsible for initiating recruitment and sent to the Dean or Director as soon as a position becomes vacant, there is an anticipated vacancy, or a new position is created. All positions which are to be filled at 50% or more time for a duration of more than six months, independent of funding source, must be opened for recruitment by an approved Form A.
A Form A is not required for Instructional Academic Staff hired from groups position funds for one semester or less OR at 49% time and less. A Form A is also not required for student assistants and employees-in-training (those defined as "Other" under Chapter 16 of the Wisconsin Administrative Code) or classified limited term employee (LTE) appointments.
Following approval of the Form A by the Dean or Division Head, the From A is submitted to the Department of Human Resources.
