
This is a sample of the document which is to be sent by recruiting departments to all applicants for UNCLASSIFIED positions. The department completes variables 1, 2, 3 and 4 and mails the form to the applicants as resumes are received. Applicants designate their preference for identify disclosure and return the form to the Chair of the Search Committee.
This form should be sent to applicants ONLY if they do not have an email address and cannot be contacted via the Applicant Information Tracking (AIT) System. The recruiting department must download or copy this form, and mail it to the applicant when the resume is received.
