This form is used by departments recruiting for Faculty, Limited Appointment, and Academic Staff positions. The form is submitted when the department is ready to interview along with the Form B - Interview stage. The department forwards the form to the school/college/division personnel representative who seeks campus level approvals.

The form captures gender, race, veteran status, disability, and referral source data. The department should complete only the gender, if known. It should not complete the race, veteran or disability status. Human resources will complete these boxes from information on the returned AA forms.
