Affirmative Action Information Form
( Faculty, Limited Appointment, and Academic Staff Applicant Self-Identification )


This form is used by a department recruiting for UNCLASSIFIED positions. It is used to assess affirmative action efforts and to comply with State and Federal regulations for monitoring applicant pools.

Applicant response is voluntary and the information provided is used solely for affirmative action purposes. Postage-free return envelopes addressed to Human Resources are provided for the mailing to applicants.

The form captures gender, race, and referral source data. Applicant name is optional.

This form should be sent to applicants ONLY if they do not have an email address and cannot be contacted via the Applicant Information Tracking (AIT) System. The recruiting department must complete the top portion of this form, copy it, and mail it to the applicant when the resume is received. (Please note: The top portion of this form must be fully completed in order for it to be processed by the Human Resources department.)
