Affirmative Action Information Form
( Classified LTE Applicants )
This form is used by recruiting departments for LTE Jobs. The department
completes the top portion of the form, copies it, and presents it to
applicants to complete at the time of the interview. Applicant
completion is voluntary and the information is used solely for
affirmative action purposes. The department transfers the information to
the Classified Staff LTE Candidates Log. These AA forms are filed in the
department.
The form captures gender and race information.
