Graduate Handbook - Forms Checklist

Use this checklist to ensure that you have completed and delivered all Graduate School and Department of English forms to the proper recipients. It is the responsibility of the student to ensure that all forms are completed correctly and in a timely manner. Questions regarding the proper use of forms can be directed to the Graduate School (Graduate School forms only) and the Graduate Program Assistant (all forms).

Forms from the Graduate School are indicated below with the letters 'GS'. Most of the GS forms are available online at: www.graduateschool.uwm.edu/forms-and-downloads/.

Most multipart forms (paper forms) are no longer available from the Graduate School.

Forms from the Department of English are noted with the letters 'ENG'. These forms are available in Curtin Hall 433.

  1. General information and Certificates
    • Adult/University Level TESOL Certificate Application. (ENG)
    • Graduate Certificate in International Technical Communication (ITC) Application (see the ITC Graduate Certificate Coordinator for this form).
    • Graduate Certificate in Professional Writing and Communication Application. (see the Professional Writing Coordinator)
    • Change in Plan of Study form. (ENG)
    • Internal Application for New or Renewal Assistantship. (ENG)
    • Teaching Assistantship Resignation form. (ENG)
    • Declaration of Committee form (ENG-used at MA and PhD levels)
  2. Special Emphases
    • Special Emphasis in Professional Writing form (See the Graduate Professional Writing Coordinator for this form).
    • Special Emphasis in Rhetoric and Composition form. (ENG)
    • ESL Teaching Assistantship Application. (ENG)
  3. Master's Program
    • Independent Study form. (ENG)
    • Summary of Coursework form. (ENG)
    • Master's Graduation Application. (GS)
    • Warrant for Master's Examination (ENG - not available for student use; consult with your major professor).
  4. Doctoral Program
    • Independent Study form. (ENG)

    • Warrant for Academic Review (ENG - not available for student use; consult with your major professor)
    • Second Language Requirement Certification form. (ENG)
    • Application for Doctoral Preliminary Examination form. (GS)
    • Graduate Assistant Justification form (GS - used with Preliminary Examination form to register for 1 credit class English 990. Used by Teaching Assistants, Research Assistants, and Project Assistants only.).

    • Application for Doctoral Dissertator Status. (GS)
    • Two Chapters Completion Certification. (ENG)
    • Doctoral Dissertation Proposal Hearing Form (GS)

    • Application for Doctoral Defense and Graduation form.(GS)
    • Warrant for Doctoral Defense (not available for student use; delivered by GS to ENG)
  5. Other
    • Change of Address form (ENG Used by the Department of English ONLY)

Note: There are other forms at the Graduate School for which you might find a need. Find those at the Graduate School web address noted above.

Suggestions:

  1. In all cases, please read the forms carefully. The GS forms have a numbered sequence that MUST be followed. Failure to do so could result in the delay of processing the application and delay in your receipt of the status or consideration for which the form was designed.
  2. Because of the volume of forms received, GS forms may take up to two weeks to process. It is therefore best to plan ahead if you must utilize a GS forms.
  3. ENG forms can generally be processed quickly, but plan ahead nonetheless.
