Medical Surveillance Program for Employees in UWM's Environmental Affairs Program

The University of Wisconsin-Milwaukee has determined that it will provide annual medical surveillance for all Hazardous Waste employees in the Environmental Affairs Program. Currently the university has a contract with Columbia St.Mary's CorporateWORx to provide occupational health services.

Please see Instructions for Employees Scheduling a Hazardous Waste Medical Exam for detailed information on scheduling an exam. Additionally there are a two forms that need to be completed prior to your visit:

• Appendix C to Sec. 1910.123 OSHA Respirator Medical Evaluation Questionnaire (Mandatory)
