Great Lakes WATER Institute Hazardous Waste Pick-Up Form

In order to serve your hazardous waste needs more efficiently; the office of Environmental Affairs has created a Hazardous Waste Pick-Up order form.

Waste is be picked up by Environmental Affairs every Monday at 10:00 AM. In order for your waste to be picked up, this order form must be completed in full, before 9:00 AM on Monday. If a form is not submitted, there is no way of knowing who needs a pick up.

Also, if you request a waste pick-up, please be available during that time or designate an alternate contact person who will be available in the comments section. Thank you very much and feel free to contact Environmental Affairs at 229-2883 or email hazwaste@uwm.edu.
