Instructions for Employees Scheduling a Hazardous Waste Medical Exam
To schedule your annual Hazmat Exam, you will need to complete the:
- Occupational Health Clinic New Client Identification Form
and
- Appendix C to Sec. 1910.134: OSHA Respirator Medical Evaluation Questionnaire (Mandatory)
Take the completed forms with you to your appointment.
Your appointment should be scheduled at the Columbia-St. Mary's facility located in Suite 201 of the Orthopaedic Hospital of Wisconsin at 575 W. River Woods Parkway, Glendale, WI 53212. (See map below):
Call (414) 961-6890 and indicate that you need to schedule your annual HazMat Exam. You should plan to arrive for your appointment at least 10 minutes early.
Directions from the North:
- I-43 South to Silver Spring Drive, Exit 78
- Left on Silver Spring Drive
- Silver Spring Drive to Port Washington Road
- Right on Port Washington Road
- Port Washington Road to West River Woods Parkway (1.3 miles)
- Left on West River Woods Parkway
- The Hospital is on the corner of Port Washington Road and West River Woods Parkway
Directions from the South:
- I-43 North to Hampton Avenue East, Exit 77A (Port Washington Road)
- Right on Port Washington Road. Port Washington Road to West River Woods Parkway
- Left on West River Woods Parkway
- The Hospital is on the corner of Port Washington Road and West River Woods Parkway
