Health Insurance for International Students International students are required by UWM to have adequate health and accident insurance. If you have health insurance from your home country or through an UWM Graduate Assistantship*, please bring a copy of your insurance (in English) to the ISSS office. Most international students purchase the insurance plan administered through the ISSS office.Health Insurance Enrollment 1. Go to http://www.sas-mn.com/ 2. Under the "College" header, click on "Look up school" 3. Select "Wisconsin" from the drop down menu & click "Go" 4. Select "Univ of WI-Milwaukee--International Students" and then click "Go" 5. Select either "Purchase Online" to use a credit card to purchase online OR "Enrollment form" to download and print the form. You would then complete and mail the form with your payment to the health insurance provider, Student Assurance (address on their web site) 6. Be sure to complete all of the information requested by the form for yourself and your dependent(s) (if applicable). An incomplete application could result in a delay in your coverage. Please note: when submitting your or your dependent information, you will be asked for a social security number. If you do not have a social security number, you may use your 990-__-_____ student ID number. Leave the social security number section blank for dependents who do not have a SS#. Important: all students who need this insurance must enroll no later than the 4:30pm on 30th day following the start of the coverage period. NO EXCEPTIONS. Insurance premiums will be NOT be accepted after 4:30pm on the 30th day of the coverage period. Here are the dates of the enrollment periods for each term: Annual & Fall: 8/15/2007-9/30/2007 Spring: 1/20/2008--2/20/2008 Spring & Summer: 1/20/2008 to 2/20/2008 Summer: 5/25/2008-6/25/2008 Coverage Dates August 5, 2007 - August 4, 2008
August 5, 2007 - January 19, 2008
January 20, 2008 - May 24, 2008
